Creating a Web Form

Data collection for requests begins with setting up a web form with specific audiences, request types, and collection parameters in mind. You can start by using one of the provided templates to intake requests applicable to GDPR, CCPA, LGPD, or other global privacy laws.

To create a web form

  1. On the main navigation menu, select Privacy Rights Automation.

  2. Select Setup > Web Forms. The Web Form Templates screen appears.

  3. Click the Add New button. The Request Intake screen appears.

  4. You can create a new web form using the seeded templates, which are pre-configured with applicable fields and settings based on the jurisdiction. These web forms can be customized from their default settings.

    Description

    Global

    Select this template to create a dynamic form with built-in support for various global privacy laws.

    Brazil (LGPD)

    Select this template to create a web form with settings relevant to Data Subject Rights under Brazil’s LGPD regulations, supported with multiple request fulfillment workflow options to route requests.

    Note

    The LGPD web form template is configured with Portuguese (BR) translations for fields and response templates.

    California (CCPA)

    Select this template to create a web form with settings relevant to Consumer Rights under the California Consumer Privacy Act (CCPA).

    California (CPRA)

    Select this template to create a web form with verbiage, category groupings, and settings that could be relevant to California’s Privacy Rights Act (CPRA).

    Canada (PIPEDA)

    Select this template to create a web form with settings relevant to data subject rights under Canada's Personal Information Protection and Electronic Documents Act (PIPEDA).

    China (PIPL)

    Select this template to create a web form with settings relevant to Data Subject Rights under China’s Personal Information Protection Law (PIPL)

    Colorado (CPA)

    Select this template to create a web form with settings relevant to Data Subject Rights under the Colorado Privacy Act (CPA).

    Connecticut (CTDPA)

    Select this template to create a web form with settings relevant to Data Subject Rights under the Connecticut Data Privacy Act (CTDPA).

    European Union (GDPR)

    Select this template to create a web form with settings relevant to Data Subject Rights under the General Data Protection Regulation (GDPR).

    South Africa (POPIA/PAIA)

    Select this template to create a web form with settings relevant to Data Subject Rights under South Africa's Protection of Personal Information Act (POPIA) and Promotion of Access to Information Act (PAIA).

    Thailand (PDPA)

    Select this template to create a web form with settings relevant to Data Subject Rights under Thailand's Personal Data Protection Act (PDPA).

    Note

    The PDPA web form is configured with visibility rules for data portability and Thai translations for fields and response templates.

    United States

    Select this template to create a web form with settings relevant to Data Subject Rights under United States privacy laws.

    Utah (UCPA)

    Select this template to create a web form with settings relevant to Data Subject Rights under the Utah Consumer Privacy Act (UCPA).

    Virginia (CDPA)

    Select this template to create a web form with verbiage, category groupings, and settings that could be relevant to Virginia’s Consumer Data Protection Act.

  5. Click the Next button.

  6. Complete the fields by entering a name, selecting a managing organization, and assigning a workflow to the web form.

  7. Click the Next button. The Web Form Languages screen appears.

  8. Use the checkboxes to select the languages you want to enable for automatic translation on this web form for translation. Additional languages can be added later, but will not be automatically translated.

  9. Click the Finish button.

Request Intake Screen Reference

Field

Description

Web Form Name

Enter the name of the new web form.

Managing Organization

Select from the field which managing organization is responsible for the form.

Request Fulfillment Workflow

Enter the workflow the web form will leverage for request intake.
